Notice of Practice Closure - Fayetteville, NC
Brave Care Pediatrics NC located at 2035 Valleygate Drive in Fayetteville, NC is permanently closed effective May 27th, 2022.
Brave Care will not be able to provide you with medical care after that date, including referrals, prescription refills, or reviewing treatment plans. We will also be unable to assist with requests for medical records or documents and have partnered with a medical record customer, Cariend, to support your medical record needs.
Medical Records Preservation
Brave Care is committed to ensuring that patients and caregivers maintain access to their Brave Care Medical Records after our clinic has closed and you have identified a new provider Effective April 20th, 2022 all medical records will be managed by Cariend. Cariend is a medical records custodian who will manage the records for each patient until their 25th birthday.
Cariend is not a medical provider and does not handle billing or referrals for the closed practice. Using a custodian, such as Cariend, ensures that records remain accessible for patients and their caregivers until a patient’s 25th birthday.
Cariend may only release records with the express permission of the caregiver or patient. They are HIPAA certified and follow all standards and protocols set forth with the intent of protecting personal health information.
In order to ensure a process that is compliant to HIPAA and protects personal health information, you must follow the steps below.
Step 1: Initializing the Request
Visit https://www.cariend.com/patient-records-request/ and complete the initial form. You will receive a confirmation email from donotreply@cariend.com confirming their receipt.
Step 2: Completing the Patient Authorization Form
Complete the Patient Authorization Form which you can download from the Cariend website or from the confirmation email Cariend sends. Please be sure to read it thoroughly and complete each field. Once complete you can scan it or take a clear picture of it with the camera on your mobile phone or device.
Step 3: Making Copies of your Identification
Make copies of or take clear photos of your government issued identification (driver’s license, state ID card, military ID card, or passport).
Step 4: Uploading your Identification Copies and Patient Authorization Form
Within 3 business of completing Step 1, you will receive an email from notify@egnyte.com with an upload link. Follow the instructions to upload your completed Patient Authorization form and copies of your government issued identification.
Records are typically sent out within 5 business days of completing Step 4. In order to send your records, all forms must be properly completed. Some records may take longer to process, depending on the size of the records. If mailing records, we cannot commit to a processing time for the USPS, but most records take 3-5 business days after mailing to arrive at their destination.
Digital Records: If you provided your email address and requested to receive the records digitally you will receive a download link from notify@egnyte.com within 5 business days of the completed steps.
Records Sent Directly to another Practice: If you opted to have your records sent directly to your new provider, these will be sent via fax or mail within 5 business days of the completed steps
Medical Record FAQs
How long will I be able to access records? Records will be retained until the patient reaches 25 year of age. You can access them following the process outlined above anytime before that?
I never went to Brave Care, but was a patient of Dr. Appels. Will I still be able to access those records?
Yes, any records available from Dr. Appels will also be available via Cariend.
Who can request medical records?
Records can be requested by a patient’s legal guardian, if the patient is over 18 years of age they need to make the requests themselves.
I still haven’t received my records, what should I do?
Please reference the following timelines before following up. Please do not submit another request as that may further delay processing.
Digital Requests: If it has been more than 7-10 business days since you completed the process and still have not received your download link, please contact Cariend at 404-991-7080 or request@cariend.com. Do not share any sensitive information via email.
Printed Requests: If it has been more than 10-12 business days since you completed the process and still have not received your records, please contact Cariend at 404-991-7080 or request@cariend.com. Do not share any sensitive information via email. Please note that Cariend is not responsible for USPS delays.
Records Sent Directly to another Practice: If it has been more than 7-10 business days since you completed the process and your new practice has still not received your records, please contact Cariend at 404-991-7080 or request@cariend.com. Do not share any sensitive information via email.
What if I don’t have access to a copier or scanner?
You may use the camera on your mobile phone or device to take pictures of the form and your identification. Please make sure the photos are clear and include the entire form and identification card.
What if I do not have a computer or internet?
You can follow the steps outlined above from your mobile phone or device. If you do not have access to the internet. Cariend can be reached by phone at 404-991-7080.
Please note that HIPAA laws do not allow medical records to be released based on phone or email requests. Cariend can also be contacted via email at Request@cariend.com however, you should not send sensitive information or request documents through email.
