Brave Care Pediatrics NC, P.A. is announcing its closure as of May 27th, 2022. Brave Care will not be able to provide you with medical care after that date.
We recommend that you find another physician to take care of you as soon as possible to ensure uninterrupted care.
Medical Records Requests
We are happy to provide your medical records to you directly, or to another health care provider of your choice, free of charge. Since these records are confidential, we need your written authorization to transfer them to another provider.
Effective April 20th, 2022 we have retained Cariend, a medical records custodian, to manage the records for each patient until their 25th birthday. Cariend is not a medical provider and does not handle billing or referrals for the closed practice.
Requestors should be prepared to complete the Records Release Authorization Form and provide a copy of government issued identification, as required by the HIPAA laws. The fastest way to process requests for copies of medical records is to visit Cariend’s website and submit your contact information at the link below - you will then be sent an email with instructions to securely upload your authorization form and ID: https://www.cariend.com/patient-records-request/
If you do not have access to a computer or internet, Cariend can be reached by phone at 404-991-7080 - please note that HIPAA laws do not allow medical records to be released based on phone or email requests. Cariend can also be contacted via email at firstname.lastname@example.org however, you should not send sensitive information or request documents through email.

Why is Brave Care closing?
Brave Care is closing for business reasons, and the decision to close this practice did not come easily.
What kind of appointments or services will Brave Care offer during this time?
In order to maintain continuity of care to our patients, we must limit some appointment types. Effective April 20, 2022 until our closure on May 27, 2022 we will be limiting the following services:
- We will be unable to accept any new patients for Urgent or Primary Care services.
- We will be unable to accept walk-in patients for any service or concern.
- We must limit Urgent Care appointments to returning patients which need follow-up care from a previous appointment, or for our Primary Care patients who need urgent care.
- We must prioritize appointments which focus on continuity of care for our Primary Care patients (follow-up, continued medication management, etc).
- We will be unable to send or facilitate any new referrals.
- We will be unable to prescribe any new medications which may require ongoing management, such as medications for ADHD or mental health treatments such as depression or anxiety.
Will you still be processing labs and imaging?
Yes, we will continue to be able to process labs and imaging as needed for patients. After May 20, 2022 will no longer be able to send out any labs or imaging to third parties and must limit these to what can be processed on premises (tests for strep, flu, and Covid, urinalysis, basic blood work, and Xray).
What if I have concerns about medication refills?
Brave Care will be able to process your request for urgent medication refills up until May 20, 2022 as long as you have been seen by a provider for the relevant concern within the last 6 months. Brave Care will not be able to provide you with medical care, including medication refills, after May 27th, 2022.
What if I need a referral to a specialist?
Brave Care will no longer process specialist referrals as of April 20, 2022. For information about specialists, you may contact the North Carolina Medical Society or visit the North Carolina Medical Board website at www.ncmedboard.org for resources.
What if I have a visit after May 27th, 2022?
If you currently have a visit after May 27th, 2022 your visit will be automatically canceled. Please contact us to book a visit in advance of May 27th, 2022 if you need medical care before that date.
What if I owe a balance after May 27th, 2022?
Please contact our billing department at firstname.lastname@example.org or (910) 991-0076 prior to May 27th, 2022. After May 27th, 2022 you will receive a final statement from Brave Care.
